Family Pathways Senior Services supports seniors 60+ and caregivers with non-medical services designed to help them maintain their independence in the community as long as possible.
Companionship
Family Pathways recruits and trains friendly, reliable and trusted volunteers to help older adults maintain their independent living status. Companionship volunteers visit weekly or on a mutually determined schedule. The weekly visit provides social opportunities for relationship building. The goal of the program is to establish a trusted and mutually respectful friendship.
Seniors and their Volunteer socialize through activities such as:
Reminiscing / visiting
Sharing a meal / going out for lunch or coffee
Assisting with grocery shopping or errands
Accompany to physician appointments or pharmacy
Playing games or cards
Assisting with reading or correspondence
Outings to community events or any other activity that supports the seniors interests and independence and strengthens the relationship between the senior and their volunteer.
Social Respite
Family Pathways Social Respite program is known as "The Gathering". This program provides a safe and enjoyable social opportunity for seniors with early to mid-stage memory loss while their full-time caregivers receive a much needed break. A typical 4 hour day at "The Gathering" would consist of social hour with snacks, reminiscing, gentle chair exercises, crafts, games, music, entertainment and a nutritious lunch. A Family Pathways Senior Services Coordinator facilitates the days activities with the help of several very dedicated volunteers who partner with the clients in attendance. Because this program is a non-medical social respite, participants must be able to feed and toilet themselves independently.
Seniors will be interviewed to determine acceptance into the programs.
All volunteers will be required to have a personal interview, a background check and attend training.
Advocacy
Family PathwaysSenior Services Coordinators are able to offer knowledgeable information and assistance for seniors and their families wanting to develop a plan to maintain their independence. They are able to provide the following services:
Perform home safety assessments to identify potential issues and offer suggestions for areas of concern that could potentially affect independence.
Assist the senior and their family with creating a core plan for the future. This may include referrals to appropriate community, government, private and family resources.
Educational opportunities for seniors, caregivers and families.
